  • All pages signed and dated

  • Matching IDs

  • Notarization where required

  • Witnesses (if required)

  • Clean documents (no whiteout, no scribbles)

  • All pages included (even if blank or marked "not applicable")

And if you're mailing? Make sure you keep a copy and track it.
